2023-2024 Refund Request Form

Please complete the form to request a refund of the $90 athletic fee for an athlete who has been cut from a team. This form must be completed and submitted to the athletics office by the deadline below. No refunds will be issued past the refund date, and no refunds will be issued for
athletes who choose to quit teams. Payments cannot be transferred to other sports, other
accounts or to other students. Checks will be voided after the refund deadline.
